Subject: Declaration of Drug "Not of Standard Quality" on Testing - reg

Sir/Madam

It is hereby informed that the Drug/item has been reported as "Not of Standard Quality". The details of the drug reported NSQ is given below:

SDTL/L/SB/2026/1536

Dated-30/09/2026 | The sample does not conform to I.P. with respect to the test for "Dissolution" of Paracetamol Tablets IP 650 mg (Copy enclosed). |

In context to above, it is directed to:

1. Medical Store to immediately withdrawn stock from respective sub-stores/Dispensaries/patient care area.
2. Medical Store to immediately write a letter to vendor for replacement of stock with another batch, within 15 days or to pick up stock and return the payment made other process of risk purchase to be initiated and recover cost of testing (if any) from the vendor.
3. Stop use of the batch of the above-mentioned item immediately which has been reported as "Not of Standard Quality".
4. Initiate action as per relevant clause of Terms & Conditions of respective DG-ESIC Tender Enquiry/RC.
5. Maintained record of action taken at in respective user units.

Sir/Madam,

With reference to the subject cited above, I am forwarding herewith one Test Report in Form-13 (Report No. SDTL/L/SB/2026/1536) received from the Government Analyst, State Drugs Testing Laboratory, Jalukbari, Guwahati-14. The report declares Paracetamol Tablets IP 650 mg, Batch No. TV6122, Mfg. Date: 08/2026, Exp. Date: 07/2028, manufactured by ALPA LABORATORIES LTD, 33/2, A.B.Road, PIGDAMBER-453446, INDORE(MP), as Not of Standard Quality (NSQ).

You are therefore requested to immediately stop all distribution of the aforementioned batch and arrange to withdraw and recall the distributed stock of the said batch from all sources without delay.

Further, you are requested to furnish the following information within 7(seven) days from the date of receipt of this letter:

1. Total quantity of the drug received, along with invoice details and dates of receipt.
2. Name and full address of the supplier/manufacturer from whom the drug was procured.
3. Present stock position of the aforesaid batch currently held in your premises.
4. Any other withdrawal protocols (if any).

This is for your kind information and immediate necessary action.
