The Ministry of Labour and Employment has specified the mandatory form for the second notice of periodical medical examination to be issued by mine managers. This requirement applies to all mines under the Occupational Safety, Health and Working Conditions (Central) Rules, 2026. The notice is to be issued to any person who fails to attend a previously scheduled medical examination without reasonable cause. Mine managers are required to issue this second notice and must send a copy to the relevant examination authority. Failure to comply with the medical examination after receiving this second notice will result in the individual not being retained in employment at the mine from the date specified in the notice.

सा.का.जन. 604(अ) .—उपिीजवकािन्य सुरक्षा, स्ट्वास्ट््य और काययदिाएं (केन्रीय) जनयम, 2026 के जनयम 111 के उप-जनयम 4 के तहत मुझे प्रदत्त िजियों का प्रयोग करते हुए, मैं, उज्ज्वल ता, खान मुख्य जनरीक्षक-सह-सुजवधाप्रदाता, जिसे खान सुरक्षा महाजनदेिक के रूप में भी नाजमत ककया गया है, इसके द्वारा, आवजधक जचककत्सा परीक्षा की दूसरी सूचना के रूप को जनर्दयष्ट करें, खान प्रबंधक द्वारा िांच ककए िाने वाले व्यजि को कदया िाएगा, यकद वह ककसी भी उजचत कारण के जबना जचककत्सा परीक्षा में भाग लेने में जवफल रहा।
काययक्षेत्र: यह फॉमय उपिीजवकािन्य सुरक्षा, स्ट्वास्ट््य और काययदिाएं (केन्रीय) जनयम, 2026 के जनयम 111 के उप-जनयम 4 के अनुसार ककसी भी उजचत कारण के जबना जचककत्सा परीक्षा में भाग लेने में जवफल रहने की जस्ट्िजत में िांच ककए िाने वाले व्यजि को खान प्रबंधक द्वारा दी िाने वाली आवजधक जचककत्सा परीक्षा की सूचना को जनर्दयष्ट करता है।
प्रयोज्यता: नोटिस का यह रूप एक खान प्रबंधक द्वारा ककसी भी उजचत कारण के जबना जचककत्सा परीक्षा में भाग लेने में जवफल रहने के मामले में प्रबंधक द्वारा िांच ककए िाने वाले व्यजि को िारी ककया िाएगा और िारी ककए गए इस तरह के नोटिस को व्यावसाजयक सुरक्षा के जनयम 111 के उप-जनयम 4 के अनुसार प्रबंधक द्वारा परीक्षा प्राजधकरण को भेिा िाएगा और सभी खानों पर लागू होगा ।

उसे एतद्द्वारा कफर से सूजचत ककया िाता है कक उपिीजवकािन्य सुरक्षा, स्ट्वास्ट््य और काययदिाएं (केन्रीय) जनयम 2026 के जनयम 111 के उप-जनयम 4 के तहत पर जचककत्सा परीक्षा के जलए स्ट्वयं उपजस्ट्ित होना होगा................ G.S.R. 604(E).—In exercise of the powers conferred on me under sub-Rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026, I, Ujjwal Tah, Chief Inspector-cum-Facilitator of Mines, also designated as the Director General of Mines Safety, hereby, specify the form of the second notice of medical examination shall be given by manager to the person to be examined, in case of he failed to attend medical examination for without any reasonable cause.
Scope: This form specify the notice of periodical medical examination to be given by manager to the person to be examined in case of he failed to attend medical examination for without any reasonable cause as per sub-rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026.
Applicability: This form of the notice shall be issued by a mine manager to the person to be examined, in case of he failed to attend medical examination for without any reasonable cause and such notice issued shall be sent to examination authority by mine manager as per sub-rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026 and applicable to the all mines
