संिोधन करने के जलए जनम्नजलजखत जनयम बनाती है, अथायत्: - (1) इन जनयमों का संजक्षप्त नाम जिजनधानकताय जिक्षा और संरक्षण जनजध प्राजधकरण (लेखा, संपठरक्षा, अंतरण और प्रजतदाय) संिोधन जनयम, 2025 है। (2) ये 6 अक्टूबर, 2025 से प्रभािी होंगे। 2. जिजनधानकताय जिक्षा और संरक्षण जनजध प्राजधकरण (लेखा, संपठरक्षा, अंतरण और प्रजतदाय) जनयम, 2016 के प्ररूप सं. आईईपीएफ-5 के स्ट्थान पर जनम्नजलजखत प्ररूप रखा जाएगा, संख्या जजसमें से आईईपीएफ में अंतरण दकया गया है कुल 9. *जनक्षेपागार का नाम एनएसडीएल सीडीएसएल क. * डीमैट खाता संख्या जजसमें िेयर कंपनी/बैंक द्वारा जमा दकए जाने हैं (*उपयोगकताय को डीपीआईडी और क्लाइंट आईडी दोनों के साथ सदक्रय डीमैट संख्या प्रजिष् ट करनी है) ख. बैंक खाते का जििरण जजसमें रकम जमा की जाएगी (पहले से भरी हुई) (क) बैंक खाता संख्या (ख) बैंक का नाम (ग) आईएफएससी कोड (घ) एमआईसीआर कोड अजधकतम घोर्णा मैं घोर्णा करता/करती हं दक मैं द्वारा * के संकल्प/प्राजधकरण संख्या प्राजधकृत हाँ दक इस प्ररूप को भरने और हस्ट्ताक्षर करने और यह घोर्णा करने के जलए दक इस प्ररूप की जिर्य-िस्ट्तु और उसके आनुर्ंजगक मामलों के संबंध में कंपनी अजधजनयम, 2013 और उसके अधीन बनाए गए जनयमों की सभी आि‍यकताओं को संकजलत दकया गया है, तारीख * के माध् यम से हस्ट्ताक्षर करने के जलए कहा गया है। मैं आगे घोर्णा करता/करती हं दक: इस रूप में और उसके संलग् नकों में जो कुछ भी कहा गया है िह सत्य, सही और पूणय है और कोई भी जानकारी दबाई या जछपाई नहीं गई है। सभी आि‍यक संलग् नकों को पूरी तरह से और स्ट्पि रूप से इस प्ररूप के साथ संलग् न कर ददया गया है। व्यजिक के प्राजधकृत प्रजतजनजध द्वारा घोर्णा मैं घोर्णा करता/करती हाँ दक मुझे इस प्ररूप को फाइल करने के उद्दे‍य से * प्राजधकरण संख्या * ददन/माह/िर्य द्वारा सम् यक रूप से जनयुि दकया गया है। यह प्रमाजणत दकया जाता है दक मैंने इस प्ररूप की जिर्य-िस्ट्तु और उसके आनुर्ंजगक मामलों के जलए कंपनी अजधजनयम, 2013 और उसके अधीन जनयमों के उपबंधों का अध्ययन दकया है। मैं आगे घोर्णा करता/करती हाँ दक: इस रूप में और उसके संलग् नकों में जो कुछ भी कहा गया है िह सत्य, सही और पूणय है और कोई भी महत् िपूणय जानकारी दबाई या जछपाई नहीं गई है। सभी आि‍यक संलग् नकों को पूरी तरह से और स्ट्पि रूप से इस [भाग II—खण् ड 3(i)] भारत का राजपत्र : असाधारण 7 दािेदार द्वारा घोर्णा मैं घोर्णा करता/करती हं दक इस प्ररूप की जिर्य-िस्ट्तु के संबंध में कंपनी अजधजनयम, 2013 और उसके अधीन बनाए गए जनयमों की सभी आि‍यकताओं और उनके प्रासंजगक मामलों का अनुपालन दकया गया है। मैं यह भी घोर्णा करता हं दक इस प्ररूप के संलग्नक सजहत यहां दी गई सभी जानकारी सत्य, सही और पूणय है और कोई भी महत् िपूणय जानकारी जछपाई नहीं गई है। मैं घोर्णा करता/करती हाँ दक मैंने इस कंपनी /बैंक के जलए पहले दकसी भी अिसर पर उसी फोजलयो और/या रकम और/या जित्तीय िर्य के अंतगयत दािा दकए गए िेयरों और/या रकम का दािा नहीं दकया है/नहीं करूंगा। * मैं समझता/समझती हाँ दक मैं, दािेदार/प्राजधकृत प्रजतजनजध हाँ, इस प्ररूप में ऑनलाइन प्रजतदाय दािा फाइल करने के बाद, नीचे जनधायठरत संलग्नक कंपनी/बैंक के नोडल अजधकारी (आईईपीएफ) को उसके रजजस्ट् रीकृत कायायलय में "आईईपीएफ प्राजधकरण से प्रजतदाय के जलए दािा कों" जलफाफे पर जलख कर भेज दूंगा/दूंगी तादक दािे के जलए सत्यापन प्रदक्रया िुरू दकया जा सके। 1 दािेदार हस्ट्ताक्षर के साथ सम् यक रूप से भरे हुए दािे के प्ररूप का हप्रट आउट 2 पािती मेल की प्रजत 3 क्षजतपूर्तत बांड (मूल) दािेदार हस्ट्ताक्षर के साथ 4 पठरपि जमा या जडबेंचर के प्रजतदाय के मामले में, मूल प्रमाण पत्र 5 आधार काडय की प्रजत 6 हकदारी का प्रमाण (िेयर का प्रमाण पत्र/ब्याज िारंट आिेदन संख्या आदद) 7 जनरस्ट् त की हुई चेक लीफ 8 जिदेजियों और एनआरआई के मामले में पासपोटय, ओसीआई और पीआई काडय की प्रजत 9 प्राजधकृत प्रजतजनजध द्वारा दायर दकया गया है तो प्राजधकृत पत्र की हस्ट्ताक्षठरत प्रजत 10 अन्य िैकजल्पक दस्ट्तािेज़, (यदद कोई हो) ठटप् पण: कंपनी अजधजनयम, 2013 की धारा 448 के उपबंधों की ओर भी ध्यान आकर्तर्त दकया जाता है जजसमें कहा गया है दक:- "इस अजधजनयम में अन्यथा उपबंजधत के जसिाय, यदद इस अजधजनयम या इसके अधीन बनाए गए जनयमों के दकसी भी उपबंध के प्रयोजनों, द्वारा या के जलए, दकसी जििरणी, ठरपोटय, प्रमाण पत्र, जित्तीय जििरण, जििरजणका, कथन या अन्य दस्ट्तािेज में, कोई व्यजि कथन करता है, - (क) जो दकसी भी महत् िपूणय जिजिजियों को गलत जानते हुए भी गलत बताता है; या (ख) जो दकसी महत् िपूणय त्य को यह जानते हुए दक िह महत् िपूणय है, का लोप करना हैं, तो िह धारा 447 के अधीन उत्तरदायी होगा। केिल कायायलय उपयोग के जलए: ई-प्ररूप सेिा अनुरोध संख्या (एसआरएन) ई-फामय फाइल करने की तारीख (ददन/माह/िर्य) ”. (1) These rules may be called the Investor Education and Protection Fund Authority (Accounting, Audit, Transfer and Refund) Amendment Rules, 2025. (2) They shall come into force with effect from 6th October, 2025. 2.
